Booking Doctor Appointments

Load the two forms below. After loading "Book" you will need to use the online editor to change the ident of the survey that it refers to from the "appearance" of question "q1". Set this to refer to the "Slots" survey.

Then create some slots that can be booked with various doctors. Finally use the book survey to book appointments.
